Meals Eligibility form. Each family MUST complete a form even if you do not feel you qualify. You may apply online at https://www.myschoolapps.com/Home/PickDistrict
Lunch payments: Lunch is $1.75 (unless you qualify for free lunch) and payment are to be made via myschoolbucks.com. For a instructions on how to use this website you may download the fact sheet here.
Bus Request: All families MUST fill out a bus request form. Download the form in the language you need here. English, Chinese, Korean, Spanish. Students who live north of the LIRR train tracks may qualify for bussing. Parents MUSt fill out the Bus Hazard form and mail it to OPT on their own. The school cannot do this for you. Please download the BUS HAZARD form here fill it out completely and mail it out. Please remember to follow up with OPT at the number on the form in the last week of august. Please do not cal the school as no one is available to help you over the summer.
Moving Forms: If you are moving you must notify the school and give proper information so we may discharge and send your records to the new school. Please download the form, fill it out and bring it to the main office.
Traveling forms: If you intend to travel with your child for more than 2 days when school is in session please fill out the form and bring your travel documents to the school
School Shirt: Red Shirts are REQUIRED to be worn for school trips and gym. Show your school spirit by wearing your PS 94 school Shirt. Order forms can be downloaded here

If you are planning to travel and your child will miss school more than 5 days YOU must notify the school. Print out ad complete an "Intent to Travel" form complete it and send it in to your child's teacher along with proof of travel such as plane tickets or travel documents. Download the form here.
If you are moving out of the area and switching schools, you must send in a notice of "Intent to Move". This tells us where and when you will be switching schools and where to send your child's records. Without this form we can not forward information to your new school The form is available here.
